5 questions to ask before discharge

If someone you love is coming home from the hospital or rehab, here are 5 questions worth asking before discharge:

1. What help will they need in the first 72 hours?
2. What equipment should already be at home?
3. Which symptoms are expected, and which ones mean we should call?
4. Who do we call after hours?
5. What appointments, medications, or instructions should be written down before we leave?

The best time to ask is before everyone is tired and standing in a parking lot with a folder full of papers.

Bathroom safety scan

A 10-minute bathroom safety scan can prevent a lot of stress.

Look for loose mats, poor lighting, slippery floors, towels that are too far away, and anything that requires reaching or twisting. If someone is rushing to the bathroom at night, add a soft night light and clear the path from bed to bathroom.

Small changes are still changes. They count.

C / Family conversations

When a parent refuses help

A gentle script for when a parent refuses help:

'I know you want to stay independent. I want that too. I am not trying to take over. I am trying to make the day safer and less stressful. Could we try one small change for a week and see how it feels?'

Sometimes the goal is not agreement forever. It is permission to try one next step.

Simple morning setup

One caregiver trick that helps: set up tomorrow morning tonight.

Put clothes, glasses, water, shoes, medications, and the first task in plain sight. Mornings are often harder when there are too many choices, not enough light, or too much rushing.

A calm routine is not about control. It is about making the day easier to begin.

The caregiver needs support too

A quiet reminder: the caregiver is part of the care plan.

If you are tired even after sleeping, more irritable than usual, avoiding people because explaining everything feels too hard, or feeling guilty whenever you rest, that is information. It may be time to ask for one specific kind of help this week.

You do not have to wait until you are completely depleted.

It can be tiny: a night light, a better chair, a new phrase that worked, a neighbor who checked in, a calmer morning routine, or finally moving the laundry basket out of the hallway.

Small wins are still wins.

G / Home safety

The path from bed to bathroom

One of the most important safety checks is the path from bed to bathroom.

Tonight, look at it in low light. Are there shoes, cords, baskets, rugs, or furniture edges in the way? Is there a lamp or night light within easy reach? Can a walker or cane move through without catching?

This is a small check, but it protects a very common moment.

H / Memory changes

Less correcting, more cueing

With memory changes, correction can sometimes create more stress than clarity.

Try a cue instead: 'The bathroom is this way,' 'Your sweater is on the chair,' or 'Let's do breakfast first.'

You are not giving up on truth. You are choosing the shortest path to calm.

I / Mobility

Before helping someone stand

Before helping someone stand, pause for three checks:

Are their feet flat on the floor? Are their glasses, shoes, and walking aid ready? Do they know where they are going next?

A rushed transfer can turn into a fall. A 10-second pause can change the whole moment.

J / Family conversations

Ask for a task, not general help

If you need family support, try asking for one specific task instead of general help.

'Can you handle pharmacy pickup every Thursday this month?'
'Can you call Dad on Sunday evenings?'
'Can you take the first appointment ride in May?'

Specific asks are easier to answer, easier to remember, and easier to share fairly.

K / Daily routines

The two-minute reset

When the day starts to unravel, try a two-minute reset.

Lower the noise. Offer water. Sit down if everyone is standing. Use one short sentence. Choose the next tiny step, not the whole day.

Caregiving often asks for big strength. Sometimes the most useful strength is slowing the room down.

L / Home safety

Rugs are emotional

Rugs can be surprisingly hard to talk about.

They may be beautiful, familiar, or connected to family history. But curled edges and sliding mats can also be fall risks.

If removing a rug causes conflict, try starting with one high-risk path: bed to bathroom, bathroom to kitchen, or favorite chair to front door.

The folder is not the plan

A discharge folder is helpful, but it is not the whole plan.

Before leaving, ask someone to explain the first day at home in plain language. What happens first? What can wait? Who should be called if something feels wrong?

A plan should survive a tired caregiver reading it at 9 pm.

The invisible work list

Caregiving includes a lot of invisible work: noticing changes, remembering refills, tracking appointments, watching mood, managing laundry, anticipating meals, calling offices, and making decisions no one sees.

If you are exhausted, it may not be because one task is huge. It may be because the list never fully leaves your mind.

The goal is not to make the home look clinical. The goal is to make the home easier to live in.

A small fall-prevention habit: keep safe shoes or non-slip slippers by the bed.

Many risky moments happen before the day officially starts. Bare feet, loose slippers, low light, and urgency can be a bad combination.

The best safety tool is often the one waiting exactly where it is needed.

A calmer family update

A calmer way to update family:

'Here is what changed. Here is what is stable. Here is what I need help deciding. Here is one task someone else can take.'

Updates go better when they are not just a long list of stress.

Reduce choices

When someone is tired, in pain, or confused, too many choices can make care harder.

Instead of 'What do you want to wear?' try 'Would you like the blue sweater or the gray one?'

This is not infantilizing. It is reducing the load of the moment.

A script for accepting help

If someone says 'Let me know if you need anything,' it is okay to answer with something specific.

'Could you bring dinner Tuesday?'
'Could you sit with her for one hour Saturday?'
'Could you pick up the prescription?'

People often mean it. They just need a door they can walk through.

The favorite chair test

Try the favorite chair test.

Can the person stand up from it safely? Is the phone nearby? Is there water within reach? Is the walking aid close enough? Is the path to the bathroom clear?

The chair where someone spends the most time deserves a safety scan too.

One phone number list

After a hospital stay, make one phone number list before the papers scatter.

Primary doctor. Specialist. Home health. Pharmacy. After-hours line. Transportation. Main family contact.

Put the list where the caregiver will actually look, not where it looks neat.
